Your Opportunity The Together Home Program is a key initiative to support the Premier’s Priority to halve street homelessness by 2025. This role will provide assistance to clients through individualised and holistic client centred support to explore and sustain housing options, financial stability, mental health/AOD support and referrals as required. Your key responsibilities will be to:
  • Conduct comprehensive assessments to identify client’s needs, strengths and goals
  • Develop individualized case plans in collaboration with clients, focusing on housing stability, employment, healthcare and other essential needs
  • Provide ongoing case management support including advocacy, crisis intervention and referrals to community resources
  • Collaborate with internal teams and external stakeholders to coordinate services and ensure holistic support for clients
  • Maintain accurate and up-to-date documentation of client progress and service supervision
  • Participate in team meetings, trainings, and professional development activities as required
Requirements for Success
  • Bachelor's degree in social work or Diploma in community services with minimum of 3 years’ experience in related field
  • Previous experience working with individuals experiencing homelessness or in a similar social setting
  • Strong interpersonal skills with the ability to build rapport with clients
  • Ability to work independently and as part of multidisciplinary team
  • Excellent communication skills
  • Compassionate, empathetic and nonjudgmental approach to working with clients
  • Driver’s license and WWCC
