We believe in empowering our consumers to live their lives with dignity and choice every day.About the role:Working in collaboration with the Care teams including Care Managers, Care Coordinators and Allied Health team, the AN-ACC and Documentation Officer will undertake accurate assessment, documentation and auditing to achieve quality care outcomes for all Residents. While playing a crucial role in ensuring high-quality person-centered nursing practices and maintaining accurate documentation, the AN-ACC and Documentation Officer will craft meticulous assessments, care plans, and documentation to reflect customer journeys, needs, and regulations, while effectively managing the AN-ACC model to optimise funding allocations and compliance.
As an AN-ACC and Documentation officer, you will actively lead, support, mentor and educate multidisciplinary teams on AN-ACC assessment, clinical documentation and practices that supports the requirement of the AN-ACC funding model. About you:You are collaborative, result oriented, determined and passionate about Aged Care. You are a Registered Nurse Australian Health Practitioner Regulation Agency (APRHA) with no restrictions on practice.
Bachelor's degree in Nursing or equivalent
Previous residential aged care experience - highly regarded
Understands the requirements of the accreditation system.
AN-ACC or ACFI experience
Experience working with electronic clinical documentation system, such as Leecare.
Experienced Registered Nurse
Have current/recent experience in, and awareness of, contemporary issues in aged care.
Commands superior knowledge of the processes related to AN-ACC and the associated business rules.
Candidates may be subject to a pre-employment medical assessment with the successful candidate requiring current NDIS worker clearance. In accordance with current health guidelines for the aged care sector, candidates are encouraged to have all recommended vaccinations.Why LHG?
