Western Virtual Service Delivery Directorate About the role The Registered Nurse - Integrated Care Coordinator will operate within a dynamic, supportive Health District wide Care in the Community team, collaborating with clients, carers, General Practitioners and service providers across all sectors of health and social care. Your role will involve delivering comprehensive holistic assessments, connected and coordinated care to people in our Integrated Planned Care for Better Health and ED to Community programs, operating under Western Virtual Service Delivery Directorate. If you have a background in Mental Health, chronic disease or coordinating care for people with complex health needs then this may be the job for you.
