The Specialist Palliative Care Service at the Mackay Base Hospital is seeking applications for a Bereavement Coordinator (Senior Psychologist or Senior Social Worker) to join their team on a permanent basis.

Your opportunity
To deliver high level comprehensive bereavement services, and high-level Social Work/Psychology clinical skills and leadership to provide effective and efficient personalised grief and bereavement care to patients and their families engaged with the Specialist Palliative Care Service multi-disciplinary team. Clinical service provision will to patients, their families/carers and other key stakeholders will be via home visits, outpatient appointments, inpatient appointments and telehealth.
About the Team
The Mackay Hospital and Health Service Specialist Palliative Care Service aims to improve the quality of life for patients and their families facing the problems associated with life-limiting illness. This can be achieved through the prevention and relief of suffering by means of early identification and treatment of pain and other problems, in the physical, psychosocial and spiritual domains. The Specialist Palliative Care Service seeks to:
* Provide holistic patient-centred care for patients with life-limiting illness across the care continuum.
* Provide specialist palliative care for patients with life-limiting illness which are needs-based regardless of diagnosis or prognosis.
* Address the specialist palliative care needs of patients and their families for the duration of their illness trajectory.
* Provide multidisciplinary care in a reliable and efficient manner.
