About the Role

Join our vibrant, multi-disciplinary community rehabilitation team as a Social Worker and embark on a journey of dynamic goal focused group work and individualised rehabilitation. We are seeking a passionate professional to contribute their expertise in social work to drive positive outcomes for our diverse clientele. Telehealth experience is advantageous as we embrace innovation and accessibility in our practice. Group programs include but are not limited to relaxation group, memory group, cardiac rehabilitation, pulmonary rehabilitation and falls prevention.


The Community Rehabilitation team is made up of a diverse group of professionals including Allied Health Assistants (AHAs), Dietitians, Exercise Physiologists, Nurses, Occupational Therapists, Physiotherapists, Speech Pathologists, and fellow Social Workers. You'll receive strong professional support from the Social Work department!
