The Continuing Care Team provides case management services for individuals over the age of 18, who are experiencing moderate to severe impairment in functioning due to mental illness and have difficulty accessing and maintaining social supports. The service is recovery oriented, person centred, family inclusive, collaborative and empowering; and operates on the premise that consumers can and do recover from mental illness and drug related disorders.

In addition to coordination of Clozapine clinics in the Continuing Care Teams, Mental Health and Specilised Services (MHSS), the RN Health Wellness and Clozapine Therapy Service will collaborate in clinical and organisational governance to ensure compliance in the SCHHS-MHSS with all clozapine protocols and patient monitoring systems, and metabolic monitoring policies/procedures.

Key Responsibilities include:

  • Adhere to defined service quality standards, health and safety policies and procedures relating to the work being understand to ensure high quality, safe services and workplaces.
  • Fulfil the responsibilities of this role in accordance with Schedule 2, Nurses and Midwives (Queensland Health Award) – State 2015 – Generic Level Statements Nurse Grade 5 and the Queensland Public Service values.
  • Understand your broad responsibilities for safety and quality in healthcare, actively participate in continuous quality and safety improvement activities within the workplace and notify a relevant clinical or non-clinical responsible officer when concerns exist about workplace or patient safety.
  • Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.

Salary between $82,753 - $106,144 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 5) (Permanent Full Time position).
