About the Role

The Project Manager - Virtual Medicine Models of Care will work with key stakeholders in the Upper Hume and Central Hume local area health partnerships to deliver a project aimed at establishing and scaling virtual medicine/in-reach models of care across our catchments.

This project is aligned with the key strategic priority of the local area health partnerships to enable improved patient flow. Key outcomes will include the development and scaling of model(s) of care that support:

  • an increase in the volume of patients managed through physician in-reach/virtual medicine models
  • an increase in supported or facilitated transfers between services
  • a decrease in the number of unnecessary transfers between services due to inadequate or unavailable medical support
  • optimized usage of bed capacity throughout the Central and Upper Hume local areas.

This role will require strong working and collaborative relationships with key stakeholders across multiple health services

Employment Type: This is a Fixed Term Full time opportunity through to June 30 2025
