We currently have a Permanent Full Time position available to fill.

Position Profile: We are seeking an experienced and passionate person to join our team within the Integrated Case Management Program.

As a Case Management Officer , you will provide assessment, integrated care and case management of metropolitan based clients with complex psycho-social needs and challenging behaviours presenting a high-risk of exposing others to the risk of HIV infection. Reporting to the Program Manager, your key responsibilities will include:

  • Provide management of clients with complex psychosocial needs considered to be at high risk of transmitting HIV and regular case conferencing and consultancy advice to public health and primary care providers involved in case management in non-metropolitan regions.
  • Provide outreach based program that delivers confidential counselling, health information, support, referral and follow-up; including HIV specific education to clients and their significant others.
  • Liaise with a range of service providers and clinicians to plan, implement and monitor care plans for clients.
  • Provide State-wide training and development to workers in primary health care and public health services on case management plans and case support.
