About the Role:
As a Registered Nurse , you will work autonomously providing high-quality person-centred care supporting NDIS participants across various community settings.
Key Responsibilities:
- Delivery of evidence-based services
- Development of management plans for high-care community needs
- Provision of up-to-date reports for participants
- Collaboration with health professionals to ensure a holistic healthcare approach
- A minimum of 5 years' nursing experience (NDIS / Aged Care / Community Nursing / Hospital sectors)
- Excellent clinical skills, including the ability to conduct assessments
- Experience in Continence and/or Complex Wound management
- Previous experience working with participants funded by the NDIS (desirable)
- Exceptional organisational and time management skills
- Reliability and willingness to travel
- Current AHPRA registration as a Registered Nurse
- Australian working rights
- Police Check clearance
- Working with Children Check
- NDIS Worker ID and orientation certificate
- Valid driver's license and access to own vehicle
