The exciting opportunity :
- Be based South of the river with clients from Rockingham to South Perth
- Provide long term care through Home Care Packages (HCP) funding
- Visit clients in their home, applying your specialist knowledge to empower them, maximise their independence and improve their sense of well-being
- Perform thorough assessments of elderly clients, evaluating their physical health, mobility, strength, balance, and functional capabilities
- Develop, implement, evaluate and adjust comprehensive, holistic and progressive care plans
- Build productive relationships and involve clients in the decision making process for their care
- Deliver advice, education and training to clients and their families

About you :
- Fully registered as a Physiotherapist with AHPRA
- Must have experience as a Home Care Physiotherapist and a passion for working with the elderly
- Demonstrated ability conducting thorough assessments and creating holistic care plans
- Be confident and relish the opportunity to work autonomously and use your initiative
- Excellent interpersonal and customer service skills with the ability to quickly build rapport with clients and families
