Purpose of Position

We are looking for a talented and compassionate Case Manager with at least 2 years’ relevant experience to join our the team at our supportive and quality-focussed workplace. As a Case Manager, you will play a vital role in supporting our clients and ensuring they receive the best possible care and attention.

Responsibilities:

  • Work with clients to assess their needs and develop a personalised care plan.
  • Coordinate and monitor services and support for clients.
  • Monitor and review client progress and make adjustments to care plans as needed.
  • Advocate for clients and ensure their needs are met.
  • Collaborate with other team members to provide comprehensive care.
  • Liaise with other healthcare professionals and community services to ensure continuity of care.
  • Ensure compliance with relevant policies, procedures, and legislation.
  • Maintain accurate and up-to-date records and documentation.

Requirements:

  • At least 2 years’ experience in case management in the Health, Aged Care or Community Services sectors; OR a formal qualification (Certificate III/IV or Diploma) in Community Services or Counselling. AHPRA registration as an RN or Allied Health Professional will be considered
  • Experience working within or demonstrated understanding of the Aged Care Quality Standards framework.
  • A National Criminal History Check / Police Check showing no disclosable court outcomes or convictions, and issued within 3 years.
  • A current, valid Australian Driver Licence.

Desirable:

  • Tertiary qualification in Social Work, Aged Care, Disability Services, Community Services Coordination, Nursing or Allied Health discipline with a current AHPRA registration, or equivalent experience.
  • Experience working in case management in either Home Care Packages or Community Home Support Program.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Spanish speaking.
