Due to an increase in demand for our services, we have ongoing multiple-positions available for this role in Tennant Creek, NT.
Position Description
As a family support worker, you will assist the work of Social Workers and Welfare Workers by providing services and support to families.
Tasks include:
(a) Interview clients and assess the nature and extent of difficulties
(b) Engage with children, families and young people and work alongside the practitioners to promote the safety and wellbeing of children.
(c) Provide practical support to children, families and young people, under the guidance and supervision of a Teaml Leader, to help them achieve positive outcomes.
(d) Liaise with relevant practitioners and senior practitioners and services in relation to the provision of case support services.
(e) Assess clients’needs and planning, developing and implementing educational, training and support programs
(f) Engage with families and work together to develop effective plans for long-term positive outcomes
(g) Engage families in their homes and at other suitable locations, to actively and proactively overcome barriers to engagement
(h) Create and update individualised case management files for all clients
(i) Connect families with other services through referrals out
(j) Support families and provide education and care for children and disabled persons in adult service units, group housing and government institutions
(k) Assist young people to solve social, emotional and financial problems
To be eligible, the ideal candidate must have/be willing to obtain the following:
Relevant AQF diploma or higher obtained in Australia and one year of full-time work experience or relevant overseas qualification equivalent to an AQF diploma and 2 years of full-time work experience or three years of full-time work experience
Current National Police Check
NDIS Worker Screening Check
First Aid Certificate
Availability to work across a 24/7 Roster to meet operational needs
NT Drivers' License
