The exciting opportunity:
- Work in your local community - full or part time role in Geraldton
- Conduct clinical assessments and provide clinical care to clients to support them at home
- Plan, implement and evaluate innovative health programs designed to meet client’s identified needs
- Liaise with Health Professionals to ensure client’s care needs are being met and outcomes achieved
- Work within the standards for the Home Care Packages Programme, and provide holistic care within the Case Management Framework
- Maintain accurate documentation and electronic records, in line with governing legislation
- Work independently whilst receiving guidance and mentoring from experienced senior clinicians
- A supportive, caring team environment awaits with strong clinical and allied health support

- Salary dependant on experience + salary packaging
- An AHPRA Registered Nurse and experience as a Care Manager within home care
- Demonstrated ability conducting thorough assessments and creating individualised care plans in line with client’s goals
- Fantastic customer service skills with a proven track record building and maintaining relationships with clients
- Able to work both autonomously and collaboratively with a multi-disciplinary team
- Well organised, friendly, committed and with a positive attitude
