The exciting opportunity:
  • Work in your local community - full or part time role in Geraldton
  • Conduct clinical assessments and provide clinical care to clients to support them at home
  • Plan, implement and evaluate innovative health programs designed to meet client’s identified needs
  • Liaise with Health Professionals to ensure client’s care needs are being met and outcomes achieved
  • Work within the standards for the Home Care Packages Programme, and provide holistic care within the Case Management Framework
  • Maintain accurate documentation and electronic records, in line with governing legislation

  • Salary dependant on experience + salary packaging
About you:
  • An AHPRA Registered Nurse and experience as a Care Manager within home care
  • Demonstrated ability conducting thorough assessments and creating individualised care plans in line with client’s goals
  • Fantastic customer service skills with a proven track record building and maintaining relationships with clients
  • Able to work both autonomously and collaboratively with a multi-disciplinary team
  • Well organised, friendly, committed and with a positive attitude
