The exciting opportunity:
- Work in your local community across wider Geraldton area
- Responsible for the home care coordination of over 50 clients, promoting healthy ageing and increasing their ability to stay in their home
- Coordinate assessments, care plans and reviews for support programs
- Ensure seamless coordination and delivery of customer centric quality care services
- Manage the timely and accurate completion of reports and documentation to be in line with requirements, administrative and communication systems
- Monitor and manage client budgets ensuring they comply with their approved package levels and individual requirements
- Your choice of full time or part time from 45 – 60 hours per fortnight

- Salary dependent on experience + super + salary packaging
- Certificate IV Community Services or AHPRA registration as Enrolled Nurse
- Strong understanding of Home Care Packages and Commonwealth Home Support Program including the funding elements
- Fantastic customer service and negotiation skills with a proven track record building and maintaining relationships with clients
- Committed to using a client directed model of care to empower clients and encourage reablement and community engagement
