The exciting opportunity:
  • Work in your local community across wider Geraldton area
  • Responsible for the home care coordination of over 50 clients, promoting healthy ageing and increasing their ability to stay in their home
  • Coordinate assessments, care plans and reviews for support programs
  • Ensure seamless coordination and delivery of customer centric quality care services
  • Manage the timely and accurate completion of reports and documentation to be in line with requirements, administrative and communication systems
  • Monitor and manage client budgets ensuring they comply with their approved package levels and individual requirements

About you:
  • Certificate IV Community Services or AHPRA registration as Enrolled Nurse
  • Strong understanding of Home Care Packages and Commonwealth Home Support Program including the funding elements
  • Fantastic customer service and negotiation skills with a proven track record building and maintaining relationships with clients
  • Committed to using a client directed model of care to empower clients and encourage reablement and community engagement
  • An energetic and enthusiastic leader who inspires and supports their team
