The role:

As a Support Coordinator with Oyster Care, your role is to support participants with a National Disability Insurance Scheme (NDIS) plan to achieve their goals and increase their independence through connecting to their communities.

Essential Requirements:

1. Certificate IV in Mental Health or similar; or relevant practical experience

2. Demonstrate a commitment to the safety of children, young people and people with disabilities.

3. Excellent time management, administration and delegation skills with the ability to maintain accurate, professionally appropriate and current records and written reports.

4. Well-developed interpersonal skills (written and verbal) including the ability to engage with individuals from diverse backgrounds and cultures to explain the NDIS processes to them in plain English.

5. Attention to detail.

6. Proficiency in Microsoft Office Suite and client information management systems.

7. Be an excellent team player.

8. Ability to work independently and take initiative, as necessary.

9. Ability to prioritise, manage deadlines, multi-task, work quickly and cope well under pressure.

10. A current NDIS Worker Screening Check (or willingness to obtain).

11. Current Working with Children's Check (or willingness to obtain one)

12. Current first aid certificate (or willingness to obtain).

13. Current driver's license and willingness to use own comprehensively insured vehicle.

14. Previous experience as a NDIS support coordinator

Key Responsibilities:

· Support NDIS participants with coordination of their NDIS plans, including negotiating appropriate services and connecting with community support and mainstream services.

· Promote a client-centred approach to service provision by effectively providing clients with relevant information with the aim of maximising their locus of control and their ability to choose the services they want.

· Navigate, influence and negotiate with public sector and community-based service systems for the delivery of appropriate supports in accordance with NDIS plans.

· Ensure continuous development of professional network to be able to meet the participant needs and provides the best quality care in consultation with multi disciplinarily team from ADS and other allied health services and any relevant parties.

· Follow up and prepare for NDIA plan meetings and help participants negotiate their plans according to their goals and needs.

· Ensure participants’ intervention plans are developed and implemented within 2 weeks of the referral.

· Build NDIS participant and support network capacity to understand and navigate service systems.

· Complete discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to participant care conferences.

· Provide expert advice and consultation to NDIS participants and their families on the changing NDIS environment.

· Support NDIS participants, families, and carers to develop resilience in the NDIS participants’ network.

· Support participants in the review of their plans including annual reviews, change of circumstances or new plans that have insufficient support.

  • Build a caseload to where it reaches 35 participants and then to maintain this.
  • Build referral opportunities and attend relevant networking events to promote organisations support coordination services.
  • Build positive experiences for participants while they are engaged with our service.
  • Promote internal organisational services to the wider community.

Schedule:

  • Monday to Friday – 8am to 4pm. There will be a need to occasionally work outside of set business hours depending upon participant and related stakeholder needs.

Job Type: Full-time

Salary: $70,000.00 – $85,000.00 per year
