They offer compassionate and personalized care to individuals with diverse needs and their dedicated teams provide a range of support services, including NDIS Support Coordination, Psychosocial Recovery Coaching and Allied Health. The Role
Our client is looking for an experienced Support Coordinator to join their Campbelltown team on a permanent full-time basis. The purpose of this role is to assist NDIS participants in the coordination of their supports funded in the NDIS plans. It involves monitoring and implementation of all supports in their funded plan, as well as funded supports. The goal is to ensure that participants are able to achieve their expected goals including of independence and well-being so that they are better able to participate in community. You'll be working as part of a passionate, supportive and flexible team who values collaboration and career progression.


Main responsibilities include:

  • Coordinate the day to day operation of the NDIS Support Coordination Services
  • Contribute towards participant assessment care planning, response to referrals and reviews in line with MDSS'S Policies and procedures
  • In collaboration develop and promote a team based approach to deliver participant centred care and services
  • Collaborate with participants, fostering an understanding of their needs and aspirations in order to explore options and develop actions that best meets their care needs
  • Advocate internally and externally to ensure systems and services protects the rights of the participants and give greatest participant control over decision making
  • Promote capacity building and social inclusion to maintain and promote independence
  • Monitoring and ensuring appropriate and timely resolutions of participant complaints and issues


What you need

  • Minimum 1 year experience as a Support Coordinator in a similar setting
  • Qualification in Disability Services, Aged Care or other relevant Community Services
  • Experience working with NDIS participants with knowledge of NDIS Operational Guidelines
  • Highly developed communication and interpersonal skills
  • Team player who is driven by compassion and role purpose
  • An Australian driver's license, Working with Children Check, National Police Clearance, First Aid Certificate and a NDIS Workers Screening Check
  • Own vehicle and comprehensive car insurance (desirable)
  • Psychosocial Recover Training (desirable)
