Description:
We are looking for a SUPERSTAR Support Coordinator with experience that can hit the ground running.
These successful candidates will work closely alongside our clients, their families, guardians and other support providers to deliver people-focused services and provide quality outcomes based on the participants’ freedom of choice.
This role will generally be responsible for, but certainly not limited to:
- Work with your participants and their support networks to determine how their funds will be most appropriately spent and what services and supports need to be engaged.
- Support and work with participants to find suitable supports that are the best match for them to actively pursue their identified goals and aspirations.
- Research, manage and navigate a range of opportunities to suit individual support needs across multiple providers.
- Demonstrate a consistent high quality in written and verbal communication. Strong comprehensive report writing skills is a must.
- Efficiently budget and break down participant funding to ensure a clear break down and get the most out of the plan for the participant.
What will set you apart from the rest?
- Minimum 2 years’ experience within a Support Coordinator role working alongside complex participants (desirable, but not essential)
- A solution-oriented individual who enjoys working as part of a driven, positive and collaborative team.
- Adaptable and can-do attitude to effortlessly adjust to participants' needs and their NDIS plans.
- Minimum Certificate III in Community Service or Disability
- Current and valid Police Check & WWCC or NDIS Worker Check
