About the opportunity
The Support Coordinator will ensure our customers are supported in achieving the goals identified in their NDIS plans, enabling greater independence!
In this role, you will:
- Build capacity and implement all supports in a participant's NDIS plan
- Report back to the NDIA on the outcome and goals, as required
- Coordinate support for individuals and families within their homes and in the community
- Collaborate and liaise with internal and external service providers
- Have a demonstrated understanding of the NDIS and experience managing funded plans
What we are looking for:
- Previous experience building effective relationships with members of various communities, including A&TSI and CALD communities.
- Demonstrated experience in interpreting and implementing NDIS plans
- Experience working with people with disability and their support networks
- Demonstrated experience in providing support using enablement and strengths-based approaches
- Tertiary qualifications in a relevant field
- Driver's licence: travelling within the region will be required
- NSW Working With Children Check or willingness to obtain
- NDISWC
- Experience using the Carelink system will be highly regarded.
