Salary: ASO2 $57,842 - $62,221 p.a. (Pro-rata)
Multiple Temp F/T & P/T positions available working up to 27/12/2024, over a 7 day roster (P/T hrs to be negotiated)
Overview
Accountable to the Program Manager, Country Referral Unit ,the Intake Worker is the first contact for clients requesting community based health and home care services, including Transition Care Packages (TCP), Short Term Restorative Care (STRC), Country Home Link (CHL), Rapid Intensive Brokerage Services (RIBS) and Commonwealth Home Support Program (CHSP), referrals throughout Regional South Australia, on behalf of the 6 Local Health Networks – Barossa Hills Fleurieu (BHFLHN), Eyre and Far North (EFNLHN), Flinders and Upper North (FUNLHN), Limestone Coast (LCLHN), Riverland Mallee Coorong (RMCLHN), and Yorke and Northern (YNLHN).
The role of the Intake Worker is to identify the clients’ initial needs through intake assessment, provide them with information on service options, determine eligibility for health services and allocate the referral to the appropriate service or worker within the region. The position will work within a primary health care framework and contribute to the service being culturally appropriate.
