Our Client is seeking a skilled and experienced Registered Nurse with 2 - 3 years experience to join their team in Sydney Olympic Park, NSW. As a key member of the Operations Department, you will play a pivotal role in delivering exceptional clinical care and client education.

Summary of Role: As the Registered Nurse, You will collaborate with clients and/or their representatives to plan and implement effective care strategies, ensuring optimal clinical outcomes. Building strong relationships with stakeholders, both internal and external, will be essential to promote effective communication and service delivery aligned with individual client needs.

Main Responsibilities:

• Lead and deliver holistic clinical and nursing care, prioritizing client needs and rights.
• Provide clinical guidance and support to the Case Management team.
• Review clinical reports and provide feedback to optimize care plans.
• Advocate for clients to ensure they receive necessary services and support.
• Conduct relevant clinical audits and assessments as required.
• Identify and mitigate clinical risks to prevent adverse outcomes.
• Facilitate training sessions to address skill gaps and ensure compliance with healthcare policies and procedures.
• Collaborate with healthcare providers to ensure continuity of care and high-quality services.
• Maintain accurate documentation of healthcare activities and consumer information.
• Monitor client progress and provide appropriate clinical follow-up.
• Stay updated on changes in regulations and laws related to home care services and implement necessary changes.

• Regular review and update of clinical assessments

• Management of consumer incidents, hazards, and injuries

Requirements:

• Current Australian Health Professional Registration Authority for a Registered Nurse
• Valid Australian driver’s license
• Bilingual in English and Mandarin
• 2-3 years’ experience in a community nursing role within the aged care sector
• Experience in coordinating assessments & care planning
• Strong computer/IT systems skills including Google Office

Additional Information 

• A number of probity checks are required including holding a current working with children and undertaking a National police checks.
