Qualifications & Experience

  • Minimum of 1 year working as a Support Coordinator OR 9 months working in a case management capacity with 3 months experience in Support Coordination
  • Proficient knowledge of the NDIS service system
  • Diploma-level qualification in a relevant area of study or evidence of enrolment
  • Understanding the housing and mental health service systems, including knowledge of available supports and pathways for participants.
  • Demonstrated ability to engage in motivational interviewing techniques with clients, facilitating effective communication and supporting the development of SMART action plans.
  • Proven ability to overcome barriers to effective client engagement, employing strategies to foster trust, rapport, and collaboration.
  • Demonstrated ability to engage in dispute resolution, resolving conflicts or disagreements in a constructive and respectful manner.
  • A proactive personal and professional development approach, actively seeking opportunities for growth, learning, and staying updated with industry trends and best practices.

 

Tasks & Responsibilities

  • Manage a caseload of 12 clients while providing person-centred support that respects individual needs and goals.
  • Support clients in engaging with representatives from other service systems, promoting collaboration and coordination to enhance outcomes and ensure comprehensive support.
  • Demonstrate the ability to effectively respond and provide support in crisis situations, employing a trauma-informed and person-centred approach.
  • Develop and implement client-specific documentation that promotes a person-centred and empowering approach to support.
  • Ability to maintain accurate and comprehensive records in compliance with relevant standards and regulations.
  • Assist clients in accessing secure and sustainable housing, leveraging mainstream or NDIS pathways while considering their unique circumstances and preferences.
  • Generate high-quality Support Coordination reports that are accurate, timely, and aligned with best practices and standards within the disability sector. 
  • Empower and support participants in building their capacity to monitor plan expenditure, make informed decisions, and exercise choice and control over their support.
  • Guide and support clients through the Guardianship process, ensuring their rights, preferences, and well-being are respected.
  • Establish connections and link clients with eligible support across various sectors, including Health, Justice, Education, Aged Care, Child Protection and Family Support, Employment, Mental Health, and Housing, as relevant, fostering a network of comprehensive and integrated services.
  • Implement an emphasis on Capacity Building and proactive planning.
