Occupational Therapist
About the role
As a full time Occupational Therapist you will be providing high-quality treatment to our patients to ensure the provision of optimal health outcomes. This encompasses ensuring effective and efficient assessment, treatment planning, treatment implementation and evaluation in a multi-disciplinary setting. You will work across a defined region providing care for our elderly or disabled community in the comfort of their home with our home visiting service.
In this role you will be…
- Ensuring appropriate delivery of quality Occupational Therapy services tailored to the individual needs of the client and using a person-centred approach
- Conducting assessments relating to Occupational Therapy and management of patients and clients in their own home
- Engaging with other Allied Health Professionals, GP’s and other Health Care Professionals to provide effective care to patients
- Writing reports such as initial reports, progress and final reports and treatment management plans which meet the standards of funding bodies such as NDIS, DVA, Medicare and HCP Providers
- Creating and maintaining effective relationships with Case Managers, GP’s, community centre’s and other referrers
