Whilst we provide skilled support services to participants with varied ages and disabilities, Create Disability Services specialises in supporting children and young people in OOHC through delivering skilled Support Coordination assistance. We pride ourselves on delivering tailored support through having a deep understanding of both industries (OOHC and Disability Sectors) and in turn, are making an impact in bridging the gap between both sectors to ensure that the disability support needs of children in care are identified, prioritised and best supported.

 

About the role: 

As an NDIS Support Coordinator, you will work with participants to search for, coordinate, and implement all supports in their plan, including informal, mainstream, community, and funded services and resources, whilst you actively support participants to build on their skills and work with them to increase their ability to access their local community and achieve greater independence.

Key functions: 

  • Support NDIS participants to implement their NDIS plan, including assistance to identify and select providers.
  • Work creatively to find positive outcomes.
  • Deal with crises.
  • Optimise participant plans to ensure participants get the most out of their funded support and plan expenditure remains within budget.
  • Strengthen and enhance participant abilities to self-direct their supports.
  • Assist NDIS participants in planning to prepare for their NDIS plan review.
  • Ensure clear, accurate, and timely record keeping.
  • Professional and ethically minded approach to supporting people with disabilities; and commitment to supporting and promoting the individual needs, skills, abilities, and personal goals of people with disabilities.
  • Ability to meet KPI targets relating to the billing of service delivery 
  • Whilst this is a position where you will be working from the comfort of your own home, you will be required to meet with local participants within the community as required.
  • Excellent verbal and written communication skills.
  • High IT, administrative, and computer literacy skills
  • Excellent active listening skills and ability to communicate with politeness, empathy, and tact.

 

Criteria: 

  • Previous experience as a Coordinator of Support or OOHC caseworker is highly desirable.
  • Minimum Diploma in Community Services, Mental Health, Counselling, Welfare Support, Disability Support, Social Work or other relevant.
  • Completion of NDIS Quality and Safeguards Commission Worker Orientation Module ‘Quality, Safety and You’, and provision of a copy of the certificate of completion.
  • Current Drivers Licence 
  • Valid NSW Working with Children Check 
  • Valid NDIS Worker Check (or willing to apply)
